Human-in-the-Loop Is Not Optional
The most reliable AI systems I have built or led all share one trait: a human validation layer at every decision point that carries real consequence.
This is not a limitation. It is the architecture.
Why Full Autonomy Breaks Down
There is a strong temptation in AI project delivery to pursue full autonomy. "Let the model decide." "Remove the human bottleneck." "Automate end to end." These goals sound efficient. In practice, they create brittle systems that fail silently.
In my Army work, I applied HITL validation to AI prototype decision-support solutions and improved operational decision-making reliability by 500%. The AI did the analytical work. Humans validated the conclusions before action was taken. Neither could have achieved that result alone.
At HRHS, the MAG-EHR platform uses AI to guide clinical staff through workflows conversationally. But the AI suggests. The human confirms. Every action that affects a patient record requires explicit human approval. This is not a compromise. It is the design.
The Delivery Implication
For project managers and delivery leaders, HITL is not just a technical pattern. It is a delivery requirement that must be planned, staffed, and governed.

- Budget time for validation workflows, not just model development
- Staff roles that include review and override authority
- Build feedback loops that capture why humans override the AI, not just that they did
- Design governance that tracks HITL effectiveness, not just model performance
HITL in the LEAP Framework
In LCAT (Leadership Cognition Augmentation Theory), this concept is formalized through two constructs. HAG (Human Augmentation Generation) evaluates the applicability of AI insights to leadership decisions. It is the mechanism that asks: "Is this insight actually useful for the decision I need to make?" CAA (Continuous Artificial Alignment) monitors whether decisions stay aligned to long-term strategy after they are made.
Both require a human in the loop. HAG is most active during the Orientation and Prototype phases of the LEAP delivery lifecycle, where the team is constantly validating whether the AI's interpretation is useful in context. CAA becomes critical after Branch Closure, when the question shifts from "Does this work?" to "Is it still working as intended?"
The LEAP principle is clear: applicability cannot be assessed through theory alone. You need a human evaluating real outputs in real conditions. That is not overhead. That is the architecture.
Control Where You Need It
The principle behind HITL is the same one that drives everything I build: control where humans need it, AI where it matters. Human in control of decisions, goals, and choices. AI in control of analysis, pattern matching, and detection. Seamless orchestration between the two.
Plan for it. Staff for it. Govern for it.
